Job Summary

 

Conducts intake interviews and screening of applications in order to determine applicants eligibility to enter the Adult Rehabilitation Centers programs; completes intake paperwork and prepares beneficiary records for file; ensures that beneficiary selection and admittance is in compliance with established program policies and procedures; orients new beneficiaries to the program by explaining requirements, rules, and regulations as well as by providing clothing and hair-cut slips, bed and work therapy assignments, and other issues; prepares and maintains chronological records of beneficiary activity and progress while enrolled in the program; tracks beneficiary attendance to ensure compliance with program requirements. 

 

Handles beneficiaries and/or potential beneficiary’s personal information with strict confidentiality and only shares the information with individuals in a need to know position. 

 

Collects and records pertinent information about the applicant’s background and need for rehabilitation; confirms alcohol and drug tests to determine sobriety as well as tuberculosis skin tests; confers with referral agencies concerning prospective beneficiaries and requests background information when necessary; discusses applicant cases with supervisor and/or Administrator in order to determine if applicant can be accepted into the rehabilitation program. 

 

Processes admitted applicants by orienting them to the program services, explaining program attendance requirements, rules and regulations, obtaining necessary signatures on formal agreements, initiating case records, taking inventory of clothing and personal belongings, dispensing clothing requisitions and arranging for dormitory assignments. 

 

Prepares a brief opening statement for each individual’s record; inputs beneficiary information into the computer throughout the beneficiary’s admission in the program; ensures the maintenance and security of all beneficiary files; files case records, records interview summaries, and verifies information.

 

Assists in assigning work therapy assignments for each beneficiary according to their particular skills and according to the position vacancies within the Center. 

 

Serves as liaison to courts and outside agencies in order to refer beneficiaries to outside programs; confers with Probation/Parole Officers to arrange for on-site inspection and terms and conditions of probation/parole when applicable; maintains regular contact with referral sources to/from other agencies and resources.

 

Prepares computerized listing of the Disposition of Forces, Bed Rosters, and Chronological Order of Admissions Lists and distributes the same to appropriate personnel daily; ensures the accuracy and completeness of the same; prepares required paperwork when beneficiaries are terminated from the program.  

 

Records and prepares statistical records reflecting the number of beneficiaries admitted to the Center as well as their participation in Center programs; ensures the accuracy and completeness of the same. 

 

Attends scheduled meetings as assigned. 

 

Maintain a positive work atmosphere by acting and communicating in a manner so that you get along with customers, beneficiaries, co-workers and management. 

 

Knowledge, Skills and Abilities

Knowledge of the practices and principles of providing social service assistance to the public.  Knowledge of general office practices and procedures. Knowledge of the practices and procedures of effective record keeping.  Ability to successfully interview beneficiaries, extrapolating the relevant information, to determine the eligibility of the beneficiary for admission to the rehabilitation center with in the established policies and procedures.  Ability to orient beneficiaries to the program by explaining requirements, rules, and regulations of the policy. Ability to prepare and maintain beneficiary records in an accurate and complete manner. Ability to ensure the security and confidentiality of beneficiary records. Maintain a positive work atmosphere by acting and communicating in a manner so that you get along with customers, beneficiaries, co- workers and management. 

 

Education and Experience

Two year college or technical school degree in social work or a related field and two years of progressively responsible experience performing social services, or public assistance work and experience performing clerical work, or any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.

 

Licenses and Certifications

Valid State Driver’s License

 

Physical Requirements and Working Conditions

Ability to effectively listen. Ability to meet attendance requirements. Ability to read, write, and effectively communicate in the English language. Ability to type and keypunch information into a computer. Ability to sort and file documents alphabetically and numerically. Ability to operate general office equipment including a telephone, computer, and adding machine. Duties are usually performed seated. Sitting may be relieved by brief or occasional periods of standing or walking.

 

Limited physical effort required associated with sitting, walking, standing, lifting, bending, reaching, twisting, pushing, pulling, and carrying light objects (less than 25 lbs.) 5-10% of work time.

 

Work is performed in a normal office environment where there are little or no physical discomforts associated with changes in weather or discomforts associated with noise, dust, dirt, and the like.
