Job Summary:
The Senior Quality Improvement (QI) Investigator is responsible for coordinating and conducting special investigations as well as being responsible for, and assisting and supporting various programs/departments on improvement recommendations particular to their specific needs.
Essential Job Duties:
- Under the direction of the Director of QI, conducts timely and thorough Special Investigations, including interviewing witnesses within the required timeframe as dictated by program regulations and agency policy; collecting, documenting, and preserving evidence in line with regulatory requirements; writing the investigation report; submitting the report to the Director of QI for review/approval; and final submission of the report based on agency policy.
- Facilitates and acts as a Liaison for Justice Center and other State Oversight Agencies: schedule witnesses, gather documentation, etc.
- Identifies incident trends, root cause analysis, and assists in creating Corrective Action Plans.
- Receives corrective actions/recommendations from program and submits evidence to appropriate State Operating Office and Justice Center.
- Monitors Corrective Action Plans to ensure effectiveness and continuation of plan.
- Reviews minor incident reports, ensures proper documentation and consistent accurate information and thorough data entry is submitted to various internal and external data collection systems (NIMRS, IRMA, etc.).
- Supports the work of the Director of QI related to incident reporting, audits, and other tasks and duties as assigned.
- Ensures compliance with rules and regulations pertaining to incident management and other QI activities.
- Works with QI department and programs to ensure Policies and Procedures (P&Ps) are written and revised.
- Attends all required internal trainings to maintain competence in position and attends external trainings as directed or per request, for ongoing professional growth and development.
- Complies with all federal and state regulations and agency policies pertaining to Health Insurance Portability and Accountability Act (HIPAA), false claims, whistleblower policies, and other applicable federal and state laws and regulations.
- Maintains confidentiality regarding privileged administrative and client information.
- Attends and participates in staff and other meetings as assigned.
- Performs all other duties as assigned by Supervisor.

Minimum Education Required:
- Bachelor’s Degree in Human Services or related field
Minimum Experience Required:
- Minimum six (6) years in Human Services or educational field
License/Registration/ Certifications Required:
- New York Driver’s License
Pay:
- $24.00 - $32.00/hour based on relevant applicable experience
