Pay Range: 
 

$21.37+ per hour based on relevant applicable experience
 

Job Summary:
 

The QI Specialist is responsible for coordinating quality improvement activities for various programs/departments throughout the agency.  This employee will assist the programs/departments with quality improvement goals and objectives that reflect their specific needs and will serve to ensure quality services and care provided by OLV Human Services.
 

Essential Job Duties:
 

1. Conducts timely and thorough Special Investigations (including interviewing witnesses, gathering documentation, reviewing evidence, and completes investigative report).

2. Classifies/categorizes cases upon notification from Justice Center or State Agencies. 

3. Assigns internal investigator or liaison.  

4. Facilitates and acts as a Liaison for Justice Center and other State Oversight Agencies: schedule witnesses, gather documentation, etc.

5. Identifies incident trends and assists in creating Corrective Action Plans. 

6. Receives corrective actions/recommendations from program and submits evidence to appropriate State Operating Office and Justice Center.

7. Performs internal record reviews/surveys for programs (clinical/medical record review, incident review) and collects data; generates reports; monitors status of responses to noted deficiencies.

8. Reviews minor incident and restraint reports, ensures proper documentation and consistent accurate information and thorough data entry is submitted to various internal and external data collection systems (NIMRS, IRMA, etc.).

Education: 

• Minimum Degree Required: Associate’s Degree in Human Services or related field

Experience:

• Minimum Experience Required: 3-5 years in human services or education field

Required License/Registration/Certification:

• Valid NYS Driver’s License
