Clinical Case Manager-Residential Program (Adult) 15CM1 

Gilead Community Services is seeking an attentive and recovery-oriented Clinical Case Manager to join our multi-disciplinary team in our supervised apartment program for women.

About the position:

The Clinical Case Manager for Gilead’s Portland area adult supervised apartment program works Tues - Sat 7a-3p providing case management, advocacy, educational, and supportive services in a program serving women with major mental illness, co-occurring substance use disorders and histories of significant trauma. The individual in this role carries out all phases of clinical services including intake, engagement, assessment, treatment planning, case management, referral, and discharge. This role also requires the timely completion of required documentation including assessments and other treatment planning.

Minimum Requirements:

  • Bachelor’s degree in a human service field; Master’s degree preferred
  • One year of clinical experience
  • Must maintain and provide proof of valid driver’s license and automobile insurance in good standing
  • Reliable use of personal vehicle as needed
  • Experience with trauma treatment, and co-occurring psychiatric and substance abuse disorders preferred

This is a union position and pays $18.42 - 20.18 per hour dependent on level of education.

Responsibilities:

  • Providing clinical knowledge regarding severe and prolonged mental health and substance abuse disorders through training and education
  • Working with the treatment team to manage clients’ mental health and assessing for risks, implementing risk management plans as necessary 
  • Providing recovery-oriented clinical case-management services with an emphasis on dignity, respect, and inclusion of family and other natural supports
  • Providing support, skills training and education in areas including daily living skills, nutrition, financial management and symptom management
  • Monitoring positive outcome measures and ensuring program compliance through accurate and timely data reporting
  • Completing assessments, treatment plans, and other documentation electronically and on paper in a timely manner
  • Acting with compassion, integrity, and ethics
