Full Time 10 hour days Adult Day Services Program Assistant

Job Purpose


Under close supervision of the Nursing Health Coordinator or the site coordinator  the primary purpose of your job is to follow oral and written instructions, perform routine procedures according to established policies and care plans.  Responsible for the delivery of care, for a specific number of participants, that meets the standard of care as directed per assignment and participant care plans. 

Job Functions

Performance Outcomes

 *Under close supervision of the Nursing Health Coordinator or Site Coordinator, follow oral and written instructions, perform routine procedures according to established policies and care plans. 


30%      *Responsible for the delivery of care, for a specified number of participants, that meets the standard of care as per nurse aide assignment and care or service plans.


____     *Responsible for bathing, grooming and toileting of participants.


____     *Performs rounds every two hours for toileting and change of position or as needed.


____     *Provides assistance with transfer, ambulation, positioning and lifting.


12%      *Provides range of motion as required in Min-Gym weight training and ambulation program per Rehab. Recommendation.


25%      *Serves dietary meals, nourishments and fluids, assist participants with meals and supervises, encourages participants who  requires cueing with meals, completing meal nourishment documentation in EHR, including Family-style Dining Program.


18%      Under the supervision and guidance of the supervisor or recreational therapist, will assist with recreational activities and programs throughout each program day, including transports to chapel and field trips or community integrated events .


____     *Takes care of participant's clothing, sorts and places soiled clothing in proper receptacle and laundered if needed.


5%       *Reports any change in participant's condition or behavior in a timely manner.


____     *Reports incidents/accidents to Supervisor and complete witness statement when called upon to do so.


____     *Completes required paperwork / EHR documentation


____     *Performs simple treatments.


____     *Reports suspected abuse/neglect to the Nurse or Site Coordinator.


5%       *Assists with admissions, transfers and discharge of participants.


____     *Assists with orienting new participants and family members to location & routines.


____     *Gives motivation and encouragement to participants.


____     *Participates in Team daily huddles


____     *Prepares participants for appointments, medical tests, therapy & activities.



____     *Maintains confidentiality of all participant care information.


____     *Follows infection control procedures as directed.


____     *Places participant belongings in safe locations.


5%       *Keeps work areas clean, clear & safe.


____     *Responsible for assigned fire duties and various emergency procedures.


____     *Participates in staff development programs on and off unit or site


____     *Responsible to attend required annual in-service programs; (6) hours of in-house in-service every (6) months.


____     *Performs other duties as assigned by professional staff.

Working Conditions

While performing the duties of this job, the employee is frequently exposed to high, precarious outside weather conditions; and extreme heat.  The employee is occasionally exposed to moving mechanical parts.  The noise level in the work environment is usually moderate.  Office building is well lighted and ventilated. 



Must have current / valid NYS  Nursing  Assistant Certification

High school diploma or general education degree (GED)

Six month related experience
