THINGS TO EXPECT A CASE MANAGER:

·         Role model appropriate behaviors to callers on the hotline and clients in the shelter, at all times, utilizing trauma informed principles and practices 

·         Provide case management, crisis intervention, domestic violence education, and resources and referral services to callers on the crisis hotline and to clients in the shelter. 

·        Communicate with hotel/motel and/or transportation agencies for client placement in overflow program and convey information appropriately to internal programs 

·         Be responsible for issues relating to admission and discharge, in conjunction with the programs definition and policies as relevant to program/client needs. Assign caseloads to case managers as appropriate.  

·         Perform screenings, lethality assessments, and basic needs assessments for potential clients according to procedure. Assist with the introduction of program and facility orientation to incoming clients. Assist with client intake/exit paperwork as needed 

·         Provide referrals and/or coordinating transportation for households to community services as relevant to client need. 

·         Facilitate and coordinate discharge planning transition and follow up according to plan. Monitor case managers completion of these tasks for their assigned cases.


·         Be available and responsive to individuals and families during hours on shift.

·         Assist with regular cleaning of shelter units, office/workspace, and common areas; assist with sorting/organizing donations and deliveries.

·         Facilitate team meetings as needed and suggest trainings, activities, and opportunities to promote self-care.

SCHEDULE: Monday-Friday 5pm-1am

PAY: $20.00/HR

 

 

QUALIFICATIONS:

Education:

·         High School or GED required.

·         Associate or Bachelor’s Degree, preferred.

·         Bilingual verbally in Spanish, preferred.

 

Experience:

 

  • 3+ years of experience working with domestic violence population, in shelters, or other closely related human services experience; OR relevant higher education courses may be substituted for experience.  
  • 1+ year experience working in Case Management required 
  • Experience working with domestic violence, homeless or economically disadvantaged population preferred   
  • Knowledge of community resources 
  • Knowledge and/or experience with trauma informed practices     

 

Compliance/Requirements:   

  • Class One Fingerprint Clearance Card 
  • Valid Driver’s License with reliable transportation 
  • Current Auto Liability Insurance 
  • 39 Month Motor Vehicle Record 
  • Must Be At Least 21 Years of Age
