A New Leaf continues to grow to respond to needs in our community by developing comprehensive, relevant and effective services for vulnerable and underserved families, children, and adults.

A New Leaf’s Parents and Children Teaming Together (PACTT) program is an in-home service for children and families with challenging needs. Through compassionate care, staff members provide community-based support and rehabilitative health service. This works to support positive behavior and healthy family dynamics.

Pay: $20 - $23 (depending on years of experience)

Schedule:  Various days/Time including weekends and weeknights

Location: East Valley 

Primary Responsibilities:

  • Document (verbal/written) and submit significant and routine paperwork such as progress notes, statistic reports, daily activities, upcoming events, special concerns, varied incident reports, etc, as required by the program.
  • Engages in Family Centered, strengths-based treatment planning and coordination and reports client needs to Child and Family Team.
  • Screening, assessing, develop, and implement treatment/service plans as well as Functional Behavioral Assessments (as required) under the direction of the Child and Family team. 
  • Ensuring that treatment/service planning will include a plan of discharge and estimated date.
  • Develops and maintains therapeutic relationships with clients and significant support systems.    
  • Able to meet monthly productivity expectations as determined by clinical leadership.        
  • Provides information, education and guidance to clients and support system members, advocacy, and coordination of care.
  • Meets frequently with client in their natural setting, or the setting of their choice.  
  • Serves as a positive role model/mentor for the client.


  • GED/HS Diploma with minimum of 3 years health & human services experience OR
  • Associates Degree in nonrelated field + minimum 1 year or health & human services experience + completion of related bachelor's degree program coursework or additional field related certifications OR
  • Associate Degree in related field or Bachelors in nonrelated plus minimum 1 year health & human services experience OR
  • Bachelor's related Degree


  • Must be able to pass various background checks
  • Must pass pre-employment drug test
  • Valid driver's license with reliable transportation
  • Current auto liability insurance
  • Clean 39-month driving record
  • Must be at least 21 years of age
