Job Overview
Prototypes Pasadena is a community behavioral health clinic in Pasadena. We provide outpatient behavioral healthcare to children, families, and adults. Services include case management, individual, family, and group therapy, occupational therapy, and medication support.
At our community behavioral health clinic in Pasadena our LVNs are responsible for providing medication support services to behavioral health clients across programs and age groups, partnering with program staff to coordinate care, and for working closely with our prescribing providers. Services are delivered on-site as well as in the field.
Key Responsibilities
- Provide office-based medication support services to individuals and families.
- Provide field-based medication support services as needed.
- Provide nursing support to prescribing providers
- Coordinate with multidisciplinary team in treatment planning and service delivery.
- Maintain timely and accurate documentation and time-keeping in compliance with agency, regulatory, and funding source standards.
- Utilize our electronic health record systems and other computer-based software, including Outlook, Excel, and Word.
- Meet expected service hour performance standards as assigned.
- Coordinate pharmacy services.
- Deliver medications to consumers when necessary.
- Facilitate medication education and health-oriented groups with consumers.
- Provide medication education consultations and collaboration with staff and student interns.
- Provide basic nursing services as needed.
- Attend internal and outside meetings as assigned.
- Assist in crisis and symptom management.
Education and Knowledge, Skills and Abilities
- Must be able to pass background/criminal check.
- Valid CA Driver License and automobile insurance.
- Bachelor’s Degree preferred.
- Licensed as a vocational nurse in California.
- Experience preferred serving seriously and persistently mentally ill clients with co-occurring challenges.
- Knowledge of trauma preferred.
