Title:                 Social Worker
Unit: Senior Housing
Reports to: Program Director
Status: Full Time; Regular; Non-Exempt
Location: Bronx, NY
Days/Hours: Monday-Friday, generally 9-5pm, flexibility required, on call 24 hours


Position Summary:


The Social Worker will provide ongoing clinical support to the senior residents in a congregate supportive housing building. The senior residents have histories of homelessness, involvement in the criminal justice system and may have mental health and/or substance use challenges. The Social Worker will engage with residents for supportive services, recreational activities and group facilitation to assist in meeting their social and emotional needs as well as being a liaison to outside providers when needed or requested. The Social Worker provides professional social work assistance to residents in areas related to illness, disease, hospitalization, financial and education by utilizing a variety of modalities in all areas of service. Responsibilities also include involvement in the discharge planning process when needed. In addition, the Social Worker will support the case managers in working with the senior residents.



Essential Duties and Responsibilities:

  • Provide supervision to Community Engagement Specialist, Peer Specialist/Wellness Coach and Recreation Specialist/Driver
  • Opportunity to supervise MSW internship students (if SIFI certified);
  • Group Facilitator for groups such as Financial Literacy, groups targeting the aging in place population, wellness groups that may include yoga, art, book club and others;
  • Assist Peer Specialist and Recreation Specialist in developing appropriate group content for their group activities;
  • Provide clinical support to case managers as they work with the senior residents;
  • Provide clinical and counselling support directly to senior residents;
  • Work cohesively with the RN to ensure that wraparound supports are in place to ensure that seniors are assisted with attaining and maintaining health and wellness;
  • Work with external providers to ensure quality implementation of supportive groups and services onsite;
  • Other duties as assigned.

Qualifications:


  • LMSW or LCSW required;
  • Minimum of one year experience working with the senior homeless population preferred
  • Some work or internship experience in dealing with either significant psychiatric issues, trauma or incarceration;
  • Minimum of 2 years of staff supervision experience preferred;
  • Bilingual Spanish-speaking preferred;
  • Strong computer skills, particularly in electronic health records;
  • Excellent resident-engagement and counseling skills;
  • Excellent clinical note writing and time management skills;
  • SIFI certification a plus;

Travel Requirements: May require travel to our Long Island City and Harlem locations and/or NYC trainings as necessary.
