The DeKalb Regional Crisis Center provides crisis intervention and stabilization services for adults.  The Center offers three levels of care starting with the Crisis Services Center unit that receives walk-ins, transfers from emergency rooms, and involuntary referrals for evaluation.

 

If further observation is needed, individuals are monitored in the Temporary Observation Unit for 12-24 hours.

Individuals needing a longer period of stabilization or substance withdrawal management (usually 5-7 days) are admitted to the Crisis Stabilization Unit where ongoing care includes crisis counseling, medication, and assistance with discharge planning.

Case Manager

 

Shifts: Weekends- 8:00 am – 4:45 pm (16 hours)


Responsibilities include:

  • Completes community needs assessments as assigned and documents them according to policy and procedure.
  • Ensures Individual Treatment Plans are completed timely as outlined in policy and procedure.
  • Assist individuals with short term or long-term placement (shelter, group home, inpatient hospitalization, etc.).
  • Provides skills groups and documents participation and response timely and as required by policy and procedure.
  • Participates in daily Treatment Team Meeting/daily Shift reports.
  • Collaborates with individual’s family and/or supports as needed for discharge planning.
  • Completes all discharge planning documentation timely and as outlined in policy and procedure.
  • Ensure discharge appointments are scheduled (i.e. behavioral health, medical, etc.).

SKILL, COMPETENCY AND EDUCATION AND/OR LICENSURE REQUIREMENTS

  • Must possess basic computer and typing skills. Must be able to successfully complete CPR, Mindset, and First Aid training.
  • Ability to deal effectively with conflict situations, manage aggressive behavior, and use non-violent crisis intervention and behavioral management techniques.

INTER-RELATIONSHIPS

  • Able to interface in a professional and therapeutic manner with individual clients and groups of clients. Able to communicate effectively with co-workers, internal and external clients, and the public.

WORKING CONDITIONS

  • Inpatient Residential program/Crisis Stabilization Unit.
  • May be required to work holidays or extended hours as needed.

NEEDED ATTRIBUTES

  • Must be well organized, consumer focused with excellent interpersonal and client service skills.

Minimum qualifications

  • Bachelor’s degree in helping field (i.e., Social Work, Psychology, Human Services, etc.) required.

Preferred qualifications

  • At least one year’s experience working in mental health or substance use disorder field preferred.
