Weekend Saturday and Sunday 8:00 am-8:45 pm or 8:00 pm – 8:45 am

12-hour shifts.  

 

 

JOB SUMMARY

 

The Social Service Provider is an integral part of the DeKalb Regional Crisis Center, which provides crisis evaluation, stabilization and withdrawal management to individuals with mental illness and substance use disorders. As an important member of our multi-disciplinary team, provides assessments, individual, family counseling, discharge planning and other treatment modalities within an evidence-based framework. Actively contributes to the treatment center and organization through on-going collaborative efforts.

 

DUTIES AND RESPONSIBILITIES

 

ASSESSESMENT

  1. Performs timely and thorough bio psychosocial assessment of the less complex to more complex clients and documents the assessment within agency guidelines
  2. Utilizes information from multiple sources as appropriate in the formulation of client driven treatment planning
  3. Utilizes specific assessment tools in evaluation of the client, including the Columbia Suicide Severity Rating Scale
  4. Demonstrates strong clinical judgment in assessment, application of diagnostic criteria, clinical formulations, and sound triage decisions
  5. Seeks clinical supervision as necessary

 

THERAPEUTIC COUNSELING

a.      a. Creates therapeutic relationship as appropriate with clients and families/significant others

b.      b. Conducts individual, family, counseling, and educational sessions using evidence-based interventions.

c.      c. Provides crisis intervention counseling
  d. Assesses and documents response to interventions and modifies interventions as needed

e.      e. Demonstrates clinical knowledge in mental health, substance abuse, and co-occurring disorder treatment

f.       f. Demonstrates clinical proficiency in client engagement, interviewing and recording

 

 

DEVELOPS AND PARTICIPATES IN CASE MANAGEMENT/CARE PLANNING

  1. Creates, communicates, and coordinates client treatment plans with clients
  2. Demonstrates expertise in care/planning through documentation of objectives, goals and specificity of plan
  3. Serves as resource to staff in demonstration of up-to-date knowledge of Claratel Behavioral Health, community, regional, and state resources.
  4. Assist treatment team in problem solving regarding needed resources for complex clients.
  5. Insures timely communication with providers through proper exchange of written and verbal communication per policy and procedure. 
  6. Document referrals, discharges, resources or other issues for problem resolution and/or utilization as required in agency policy.
  7. May serve as a liaison in community resource activities
  8. Completes timely authorization documentation for service provision
  9. Completes timely discharge planning documents, including the crisis and safety plan
  10. Completes follow-up telephone calls to discharged clients to assess current clinical status and connection with aftercare arrangements. 
  11. Completes the Structured Follow-up and Monitoring Note as per policy.

SKILL, COMPETENCY AND EDUCATION AND/OR LICENSURE REQUIREMENTS

 

Minimum Qualifications: Master’s degree and associate level license (LMSW, LAPC, or LMFT). Two years of related work experience in similar setting, computer proficiency and ability to multi-task. 

 

Preferred Qualifications: Master’s degree and Georgia Independent licensure as a Clinical Social Worker, Professional Counselor and/or Marriage and Family Therapist.

Able to communicate effectively with co-workers, internal and external customers and the public. Ability to interact and communicate with clients and their families effectively.

 

WORKING CONDITIONS

Works in a crisis setting. May be required to work holidays, weekends, or evenings.

Under general supervision, assess, screens, and counsels clients in an intensive outpatient mental health and/or substance abuse program to enhance, meet, or restore a client’s functioning capacity. Works with a multi-disciplinary team that provides individual and group psychotherapy and psychoeducation to adult mental health and substance abuse clients. Provides case management and develops client-driven treatment plans. Provides counseling/therapy to clients requiring complex intervention, including those who are homicidal, suicidal, or exhibit life-threatening behaviors, determines necessity for involuntary commitment and may partake in executing orders of involuntary commitment. Complete assessments, manage a case load, complete treatment plans and obtain authorization for services.

•Demonstrates documented clinical proficiency (through education, training, or experience) in client engagement, interviewing, and recording 

•May utilize advanced certification in specialty in counseling the consumer and others

 

 

 

Counsels Individuals, Groups, and Significant Others

Professional

SKILL, COMPETENCY AND EDUCATION AND/OR LICENSURE REQUIREMENTS:

Current licensure as a LCSW, LMSW, or LPC by the Georgia Composite Board of Professional Counselors, Social Workers, and Marriage and Family Therapists is required. Demonstrates competency in using evidenced based treatments for mental illness and addiction. The employee must be computer efficient and have the ability to multi-task. The employees must demonstrate initiative, flexibility, and professionalism.

 

Preferred Qualifications: Experience in a residential or outpatient setting with mental health or substance abuse programs. Experience working with Adults, children, adolescents and veterans. Experience conducting groups counseling/therapy services.
