We are seeking a highly motivated Population Health Performance Specialist to join our team.

 

The Population Health Performance Specialist will work within the Office of Value Based Performance and is directly responsible for supporting providers and clinical practice sites in achievement of quality improvement outcomes.  The individual in this position will be responsible for maximizing quality and ROI across numerous payer programs through education and delivery of quality improvement best practices.

 

The Population Health Performance Specialist will:

  • Monitor practice and provider progress with value-based contracts and analyze performance reports and data to inform decision-making, process, and program implementation, as well as the development of process interventions based on practice-level data, trends and identified opportunities
  • Deliver relevant data and recommendations to senior clinical leadership, inclusive of OVBP Directors, Practice Directors, and CU Medicine executive leadership
  • Serve as OVBP liaison with external program partners (e.g. payer partners)
  • Oversee development of provider education and support resources necessary for practice transformation to improve quality and meet the quadruple aim
  • Develop strategies and advise on clinical workflows to position the practice to realize quality-based incentives and to protect against fee schedule penalties
  • Provide feedback to and collaborate with EHR and analytics teams to ensure reports are accurate, meaningful and actionable
  • Provide assistance to providers in the use of population health management tools (e.g. Epic Reporting workbench, Quality Dashboards, HCC BPA & SlicerDicer)
  • Perform other duties and assist with special projects as assigned

 

Requires a bachelor’s degree in healthcare administration (or related field) coupled with 2-3 years relevant work history.  Advanced degree in healthcare administration or public health is highly desirable.  Must have experience in gathering information and understanding context from several sources, combined with the ability to analyze this information, draw conclusions and make recommendations.  Prior clinical education experience and the ability to affect change is required.  Must demonstrate a strong competency to comprehend clinical informatics and EHR technology.  The ideal candidate will successfully interact with providers and establish oneself as a subject matter expert within a clinical setting.  Must have intermediate-level proficiency in Microsoft Excel.  Strong PowerPoint and executive-level presentation skills are essential to success.  Must demonstrate strong project management skills to ensure sensitive timelines are met.  The ability to work cohesively in a team environment to achieve goals is key.  Must be flexible and maintain positive working relationships.
