For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens, and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness. The Open Door Psychosocial Club, our rehabilitation center that provides services to individuals with serious and persistent mental illness. The program is open to adults 18 and over. The overall goals of the program are to facilitate healthy socialization within the community, develop basic work skills, improve members’ ability to live and function independently, and reduce psychiatric hospitalizations. These goals are addressed on both an individual and group basis, with group being the primary modality. Group topics include medication management, time and financial management, relapse prevention, smoking cessation, wellness management.

STATEMENT OF THE JOB 
The Rehabilitation Facilitator is a role model of professional and vocational behavior that provides encouragement and guidance to the membership through organized program activities, group discussions, individual conversations, and case documentation.  The Rehabilitation Facilitators guide members towards realistic goal through use of Rehabilitation Plans and Progress Notes.
•    Engage members in activities while providing coverage during all hours of operation including some holidays.
•    Maintain a safe and peaceful environment including managing member conflicts, symptomology, and difficulties, while ensuring the rights, dignity, and safety of all members.
•    Maintain a caseload of approximately 18 members
•    Update information in charts as needed to maintain audit standard requirements
•    Adhere to all documentation requirements in addition to charts above.  This includes but is not limited to the appropriate completion of Inquiry Log Sheets, client participation in service, Co-Occurring Disorder screening, Client Profile forms and other agency and government surveys, and Group Logs documenting.
•    Facilitate various groups and meetings with membership working toward the goals of guiding their social rehabilitation, developing their decision-making skills, and increasing peer support.
•    Ensure that referrals are made to appropriate internal and external services as required to meet the consumer’s/families’ needs.

QUALIFICATIONS
•    BA or BS in a human service field.
•    NYC DOHMH Food Handlers Certification
•    Excellent clinical skills with at least two years relevant mental health, substance abuse or human service experience.  
•    Bilingual Spanish/English; Russian/English; Creole/English; Chinese/English; Korean/English speaking preferred but not required. 
•    Occasional lifting and/or moving of up to 10 pounds.
