STATEMENT OF THE JOB 
A Clinical Documentation Specialist works to ensure the accuracy of clinical data for the purpose of billing and rendering payment for services. The Clinical Documentation Specialist will review clinical documentation, billing information, clients’ records, and other related documents and will verify all documentation has been completed and approved for processing claims and payroll sheets. Their duties include ensuring that the records and reports accurately reflect a client’s diagnosis and the treatment that they receive. The responsibilities of a Clinical Documentation Specialist also include communicating with clinicians and support department staff to verify information. In addition, they will perform data entry tasks with the updated information. 

In this role, the Clinical Documentation Specialist is responsible for improving the overall quality and completeness of clinical documentation. Facilitates modifications to clinical documentation through extensive interaction with the clinicians. Ensures the accuracy and completeness of clinical information used for measuring and reporting outcomes. Participates in training to clinicians and provide ongoing support. Ensures that documentation practices and processes comply with applicable regulatory guidelines and requirements. Participate in special projects when necessary.
•    Follows OMH documentation regulations to ensure appropriate documentation for each service provided by the clinician is recorded in the EHR.
•    Ensures clinical documentation meets State and Federal requirements and standards.
•    Regularly and effectively communicates with clinical staff to ensure all documentation is completed with the 24hr window for salary staff and same day of service for Fee for Service staff. Including reviewing Fee for Service payroll sheets.
•    Educates clinical staff on clinical documentation opportunities that impacts the accuracy of the EHR.
•    Facilitates clarification of clinical documentation ensuring accuracy and integrity in the EHR.
•    Requests clarification of clinical documentation from the clinicians(s) on a concurrent basis as needed to ensure documentation is complete and accurate prior to billing or rendering payment for services.
•    Conducts follow-up reviews of clinical documentation to ensure points of clarification have been recorded in the client's chart. 
•    Ensures appropriate clinical documentation is in place to support diagnosis and ensure billing and payment.
•    Works with the clinical staff to continuously improve clinical documentation. 
•    Meets performance standards by setting goals and objectives, prioritizing work, and using available resources efficiently and effectively.
•    Stays abreast of the latest developments, advancements, and trends in the field of documentation improvement by attending seminars/workshops, reading professional journals
•    Ensures that work is completed accurately and according to schedule
•    Performs related duties as required.

QUALIFICATIONS
•    High School Diploma, Bachelor’s degree preferred 
•    At least two (2) years of patient-facing, clinic, or hospital experience in the areas of scheduling, admissions, treatment coordination and/or customer service
•    Strong knowledge of health insurance benefits preferred
•    Ability to meet essential physical demands of position, including frequent walking; using hands to finger, handle or feel objects, tools, or controls; sitting and standing.
•    Strong computer skills, including the ability to learn/use Electronic Health Record (EHR) software to effectively carry out essential business processes.
•    Proficient in Microsoft Office Suite, specifically Word, Excel, and PowerPoint
•    Exceptional interpersonal, problem solving and communication skills.
•    Willing to take initiative.
•    Highly flexible personality type.
