 STATEMENT OF THE JOB:

The Licensed Practical Nurse will be responsible for developing and implementing practices and protocols specific to the population served. Clients in crisis or with behavioral health needs (such as hoarding, depression, anxiety, substance use disorders) will be reached out to, engaged and treated by the Licensed Practical Nurse. The LPN will develop an individualized plan to address needs and include family, significant others and healthcare providers whenever possible. The primary goal of this employee is to support clients to monitor and maintain primary health goals. The Licensed Practical Nurse is part of the team and assists team members in understanding the needs of clients and help improve their quality of life.   
* Perform health monitoring services and assessments
* Responsible for tracking information/ deliverables for the Cabrini grant funds.
* Lead workshops for clients on issues related to health and wellness.
* Facilitate coordinated care for targeted critical/high need cases.
* Completes documentation, both paper and electronic in a timely manner.
* Provide counseling and therapy and provide in-service training and be available to consult with staff.
* Conduct and prepare assessments, case logs, documents case notes, care plans for clients.

Qualification:

* NYS Licensed Professional Nurse
* Must have certification in behavioral health clients or 3 years of experience working with the behavioral health
* Bilingual preferred (Chinese, Korean, Russian, Spanish)
