TRC Article 16 Clinic - Treatment Coordinator - Part Time
TREATMENT COORDINATOR –
ARTICLE 16 CLINIC
RESPONSIBLE TO: Site Manager
FUNCTION:
To assist program administration in the effective operation of the Article 16 Clinic. To provide treatment coordination services to Article 16 Clinic service recipients. To oversee and coordinate the delivery of health-related services to Article 16 Clinic service recipients.
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:
Treatment Coordinator must have:
·In depth understanding of Developmental Disabilities
·The ability to coordinate and oversee the implementation of Treatment Plans,
·Inclusive, clear and concise verbal and written communication skills,
·Ability to self direct, self initiate and handle a specified caseload
·Ability to work cooperatively with other disciplines
·Ability to work with a varied population
·Valid Driver’s License and driving record acceptable to TRC’s Insurance Carrier
·Physical condition or reasonable accommodation(s) commensurate with demands of the position
MINIMUM QUALIFICATIONS:
Registered Nurse: NYS licensure to practice nursing as a Registered Professional Nurse and one (1) year of professional experience working with persons with developmental disabilities. Or, Treatment Coordinator: Bachelors Degree in the Human Services field (including but not limited to, Sociology, Special Education, Rehabilitation Counseling, Psychology ) Valid NYS driver’s license and driving record acceptable to agency insurance carrier or equivalent accommodations.
DUTIES:
1.Performs and documents evaluation and treatment through the use of service plans, progress notes, six-month summaries and discharge summaries, in accordance with policies and procedures.
2.Develops training programs in health/medical care (example: self-administration of medication) and provides direct training to a specified caseload, according to treatment plans.
3.Documents individual progress at least monthly and on a semi-annual basis.
4.Is available to provide direct services and/or training to staff or collaterals, not only at the clinic but at off-site (example: SADH & Chestnut) locations.
5.Provides continuing education/in-service training for staff as needed.
6.Acts as a liaison with other agencies and departments mutually involved in service provision to individuals.
7.Recommends individuals for other services, as the need arises.
8.Is also responsible for reporting active cases of tuberculosis to the Chautauqua County Health Department.
9.Provides acute and/or emergency evaluation and nursing care to service recipients, as needed.
10.Completes and submits attendance and billing logs in a timely manner.
