The Resource Center has been providing services to persons with disabilities in the Chautauqua County area since 1958. From our humble beginnings as a gathering of parents and concerned citizens who wanted to provide educational and training opportunities for persons with developmental and intellectual disabilities, The Resource Center has grown into a comprehensive agency providing services to thousands of persons with all types and levels of disabilities.
 
The Resource Center is the largest non-governmental employer in Chautauqua County with more than 1,300 employees on its payroll.

NURSE PRACTITIONER

RESPONSIBLE TO:
Director of Health & Health-related Services

FUNCTION:To provide nursing and medical services to individuals, families and groups, emphasizing health promotion and disease prevention as well as management of acute and chronic diseases.

SPECIFIC DUTIES:

  1. To develop primary care protocols and procedures in the compliance with New York State Health Department regulatory standards.
  2. To implement primary care protocols and procedures in compliance with New York Health Department regulatory standards. Protocols and procedures may include but are not limited to: a. Physical assessment for employment. b. Physical assessments, follow-up and periodic monitoring of health care status of a specific caseload of individuals including: ordering diagnostic and laboratory tests, prescription of pharmacologic agents and treatment and non-pharmacologic therapies. c. Development of the plan of care and treatment/follow-up as required. Teaching and counseling individuals, families and groups are a major part of the practitioner’s role. d. Maintains written documentation of the patient’s medical appointment including: the subjective patient concerns, the objective observations by the practitioner, summary of the assessment and plan of care. e. Provides patient education regarding specific health issues as necessary. f. Makes referrals to other medical practitioners/specialists, laboratories, hospitals for further medical evaluation as the need arises. g. Consults on a regular basis with Primary Care physician/collaborative physician regarding specific patient issues and participates with primary care physician/collaborative physician in the review of primary care patient files to evaluate and review the type and quality of patient care. h. Monitors and implements infection control practices and assures that all practices with the primary care nit are in compliance with agency, State and Federal guidelines.
  3. Works autonomously as well as collaboratively with a variety of individuals to diagnose and manage patient’s health care problems.
  4. Provides professional supervision and advisement to primary care nursing staff including instruction in performance of job duties. 5. Oversees the scheduling of appointments and general medical office practices including medical record maintenance and billing practices.
  5. Is responsible for ordering and maintaining supplies for primary care unit, assures all equipment is maintained in proper functioning order. Monitors units of service productivity and office expenses.
  6. Is knowledgeable and can implement current first aid and/or emergency procedures.
  7. Participates on agency committees as required to provide professional guidance and advisement as necessary. Acts as health care resource, interdisciplinary consultant and patient advocate.
  8. May provide in-service education on medical health care issues as needed.
  9. Performs all other duties as requested by Director of Health Services.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES: Ability to work with adults and children, knowledge and abilities commensurate with those of general medical practice. Awareness of Primary Care Office Management and procedure, knowledge and certification in First Aid and/or emergency procedure, the ability to communicate verbally and in writing. Physical condition commensurate with job responsibilities.

MINIMUM QUALIFICATIONS: New York State Certification as Family Nurse Practitioner in Family Health. This is a position that is exempt from overtime provisions of the Fair Labor Standards Act. As an Exempt employee, incumbent will receive an annual salary commensurate with assigned responsibilities and salary classification; however, incumbent is expected and required to work as many hours as necessary to fulfill job responsibilities.
