General Responsibilities:

The Behavioral Health Clinician is a member of the Community Health Team (CHT); a multi-disciplinary team comprised of Nurse Care Managers, Community Health Workers, Care Management Outreach Workers, and Behavioral Health Clinicians. The CHT supports patients of Thundermist Health Center who are dealing with complex medical and behavioral health and/or substance issues and require a more intensive home and community-based intervention. 
Behavioral Health Clinicians are an integral part of the Community Health Team as they conduct behavioral health assessments, provide intensive case management, provide community based clinical interventions, contribute to the development of individual care plans, collaborate with staff, and make referrals as needed.

 

 

 

Required Qualifications:

1.     Master’s degree in Social Work and clinical experience in a community setting.

2.     Possess a RI professional license (LCSW or LICSW)

3.     Supervisory/Managerial Skills preferred.

4.     Provide own reliable transportation with proof of RI minimum requirements of auto insurance (including a rider to utilize your automobile for work purposes).

5.     Previous experience using an electronic health record preferred.

6.     Experience working on a multi-disciplinary team in a medical environment.

7.     Experience working in intensive community based services strongly preferred.

8.     Experience working with diverse populations and bi-lingual/bi-cultural candidates (Spanish/English) preferred.

9.     English, Spanish and Portuguese written and spoken language preferred based on practice location (West Warwick).



 

 

Physical Effort/Environment:

This position is primarily community-based with some time spent in an office setting. The position involves the following:

·       Meet with patients in the community; at their home, in the hospital, or other community settings as needed.

·   Collaboration with community health workers on care plan development and progress for new patients of community health team

·       Flexibility to travel within Rhode Island to meet the needs of the patient.

·       May require climbing several flights of stairs and carrying personal technology such as laptop.

·       General office environment; manual dexterity needed for keyboard work, writing and phone calls.

·       Demonstrated experience working with Electronic Health Records, computers and other technology and Microsoft Office applications.  

·       Use of technology for home monitoring, telemedicine and other devices, as needed.

 

 

Work Schedule Demands:

·       Full-time. Ability to work evenings and weekends as required.

 

Communication Skills:

·       Strong communication skills, both oral and written, required.

·       Strong working knowledge of health literacy skills (e.g. Teach Back)

·       Ability to relate to patients and build trusting relationships.

·   Ability to communicate effectively with co-workers, patients, family/care givers, and partner agencies.

 

Confidentiality of Information:

A high level of confidentiality.  Full access to medical records.  Must maintain all policies of confidentiality at all times and use discretion surrounding agency information and management issues.

 

Significant Job Functions:

Clinical & Direct Service Responsibilities

1.    Key member of multi-disciplinary Community Health Team; collaborates with physicians, other care team members (Medical Assistants, Nurses, etc.), hospitals, partner agencies, members of the CHT in the patient’s network of care.

2.    Meets with patients in the community, whether at their home, in the hospital, or another community setting.

3.    Delivers community based individual, group, couples and or family interventions.

4.    Transport and/or accompany patients to clinical and/or other relevant appointments, as necessary.

5.    Applies knowledge of Psychiatric Diagnosis (DSM-IV-TR & ICD-9) in order to perform and record mental status examination findings and multi-axial diagnostic formulation and billing codes with assigned patient caseload.

6.    Assists in the development, review and delivery of a patient Care Plan to include actions designed to improve the patient’s health status.

7.    Assess clinical emergencies, including risk of suicide and danger to others.

8.    Provide referrals to appropriate resources and follows-up as necessary.

9.    Obtain progress notes from outside providers involved in patient care.

10. Engage and empower patients to understand their health issues and work closely with patients to take proactive measures that will result in positive outcomes.

11. Duties can change as program requirements evolve.

 

Documentation & General Administrative Duties

1.    Perform timely and accurate record keeping in accordance with all Health Center requirements.

2.    Complete daily activity logs, behavioral health screening & progress notes, encounter forms, reports, evaluations, and insurance documentation as required by pay sources.

3.    Complete clear, specific, and behaviorally measurable treatment plans with assigned patients.

4.    Maintain cooperative relationships with Health Center employees, patients, community agencies and the public.

5.    Work in cooperation with co-workers and supervisory staff.

6.    Collaborate with team members resolving problems in a constructive manner.

7.    Maintain a willingness to listen to and respond to constructive feedback from supervisory personnel.

8.    Serve on Health Center committees and work-groups as requested.

9.    Perform other duties as assigned. Maintain timely and accurate records and documentation as required.

 

Lead/ Supervisory Responsibilities

1.    Coordinate the efforts of Community Health Workers (CHWs), working collaboratively to meet patient needs.

2.    Hire and develop CHWs as needed

3.    Manage bi-weekly timesheet approvals and annual performance evaluations
