General Responsibilities:
The BH Consultant, as member of the primary care treatment team identifies, triages, and provides consultation for assessment and behavioral health treatment planning, recommendations, and brief interventions.  The ISTP BH Consultant will have advance training in the treatment of Substance Use Disorder.  They will work alongside our team of Nurse Practitioner Fellows participating in the ISTP(integrated substance use training program) and provide them with real time Behavioral Health Support and education regarding effective treatment of patients care for by the fellows presenting with BH and SUD needs
Qualifications:
Required Qualifications:
•    Apply knowledge of the psychiatric diagnosis (DSM-5) to conduct and record mental status examination and multi-axial diagnosis
•    Knowledge and ability to independently carry out behavioral health screening, diagnosis, assessment, treatment planning, and treatment or referral of behavioral health patients
•    Strong knowledge base of medical terminology and procedures
•    Previous experience in a primary care setting, ideally in a federally qualified health center (FQHC)
•    Ability to work in a high energy, quickly paced environment and maintain a professional demeanor and attitude
•    Excellent interpersonal and communication skills with patients, their families, internal and external staff
•    Master’s degree (M.A., M.F.T., M.S.W.) 
•    Experience treating patients with comorbidities
•    Experience treating patients with Substance Use Disorder
•    Must be eligible for RI State Licensure: LICSW, LMFT, or LMHC.  
•    Computer skills including word processing and data entry 
Preferred Qualifications: 
•    English, Spanish and Portuguese written and spoken language
•    Previous experience with eClinicalworks

Significant Job Functions:
•    Provide consultation and assistance to medical providers participating in the ISTP program on the management of behavioral health concerns, specifically SUD/OUD  in primary care and help implement integrated treatment plan 
•    Manage warm hand-offs from providers to initiate screening and referrals to behavioral services
•    Participate in ongoing training in behavioral health issues  and provide consultation and training to primary care clinic staff in mental health and behavioral medicine topics including communication skills, brief interventions, counseling skills, substance use, wellness, recovery, and self-care
•    Develop and maintain collaborative working relationships with community providers, Initiate linkages, disseminate information and make referrals to the appropriate services as needed
•    Provide crisis intervention, conduct safety planning agreements
•    Conduct brief screenings to evaluate the needs of adults, adolescents, and children for behavioral health intervention, case management, or ongoing mental health or addiction treatment
•    Provide approximately 20 clinical hours, (2 admin, 1 supervision 1 provider meeting)and  16 hours of Integrated Behavioral Health Consultant open access services
•    Provide clinical and consultative treatment services that are strength based and patient- centered.
•    Coordinate with clinical providers, social service providers, and care coordinators
•    Document all aspects of the screening, diagnosis and treatment or referral in the EHR
•    Complete all required documentation for treatment planning, billing, and licensure within a timely manner
•    Participate in professional activities such as: continuing education activities, clinical conferences, meeting with representatives of community groups
•    Respond to crises, assess need, formulate disposition, and implement appropriate care
•    Ensure patient confidentiality at all times
•    Perform other necessary duties as required by Thundermist Health Center to achieve the goal of providing high quality behavioral health services
•    Participate in on-call BH coverage
Physical Effort/ Environment:
•    This is a hybrid position, allowing duties to be performed from home or at a Thundermist location.
•    Physical Demands: Requires walking, bending, sitting, standing, writing, reading, telephone use, data input into computer
•    Mental Demands: Ability to cope with continual changing priorities under potentially stressful conditions
•    Manual Dexterity Required: Ability to use a keyboard, telephone•    
Work Schedule Demands:
•    Full time, 40 hours per week.  
Communication Skills:
•    Strong communication skills, both oral and written.
•    The ability to effectively work and communicate with a multi-disciplinary team, other services providers, and patients is critical to the success of this position. 
•    Must develop and submit written reports accurately detailing service provision.
