Job Description
Job Title: Fellow, Primary Care Nurse Practitioner Reports to:  Program Director, Primary Care Nurse Practitioner Fellowship
Department: Medical, Integrated Care Management FLSA Status:

General Responsibilities:
• The Primary Care Nurse Practitioner Fellow is responsible for providing primary medical care, within the limits of the training of a nurse practitioner, to assigned patients while under clinically precepted supervision. The directed course for the first program year includes the fellow incrementally developing and maintaining primary care panel of patients with the goal of independently managing this panel by the conclusion of the first Fellowship year. The nurse practitioner fellow is a fully licensed, autonomous provider executing professional clinical judgment in the care of Thundermist patients, according to the guidelines, policies and procedures set forth by Thundermist Health Center and the Fellowship program.
Qualifications:
 
Required Qualifications:
• 1. Successful completion of MS, MSN or DNP program
• 2. RI RN, APRN and CSR Licensure 
• 3. ANCC or AANP Board Certification in the specialty of practice PRIOR TO AUGUST of the cohort year (Family Medicine and Adult Geriatric) 
• 4. Current and Active DEA 
• 5. Current and Active BLS 
• 6. Ongoing CEU to update skills/therapies

Significant Job Functions:
Clinical:
 1. Patient care – history, physical exam, anticipatory education/guidance in family medicine, management of acute and chronic illness 
2. Participating in On-Call on a rotating basis
 3. Maintenance of complete/accurate patient records
 4. Accountable for patient care provided
 5. Suboxone Waiver Training, Certification and Practice 
Administrative: 1. Participation in initial 4–6-week orientation period 2. Participation in weekly fellowship debriefs 3. Participation in weekly Didactic sessions 4. Participation in QI meetings as a fellowship team, designated by this team 5. Presentation of QI project to Senior Leadership 6. Participation in bi-weekly ECHO sessions 7. Participation in intermittent departmental and team meetings 8. Completion of weekly Reflective Journal 9. Duties specific to each Fellowship year
Physical Effort/ Environment:
• Moderate physical activity: walking, standing, and stooping. Potential exposure to bloodborne pathogens, respiratory disease, and other infectious diseases, and other biohazards material requiring universal standard precautions for medical practitioners. Manual dexterity needed for certain medical procedures. Clinical environment in ambulatory-based settings and home-based care.
Work Schedule Demands:
• 3 Precepted Clinic Days
• 1 Specialty Rotation Day 
• 1 Didactic, ECHO, QI Project, and Team Debrief Day
Communication Skills:
• Exceptional oral and written skills needed. 
• Proficient in all applicable medical terminology. 
• Bilingual capacity is a plus.
• Good interpersonal skills are needed.
